Medical Conditions

  • Cancer

  • Glaucoma

  • Positive status for human immunodeficiency virus/ acquired immune deficiency syndrome

  • Hepatitis C

  • Amyotrophic Lateral Sclerosis (ALS)

  • Tourette’s syndrome

  • Crohn’s disease

  • Ulcerative colitis

  • Post-traumatic stress disorder

  • Severe arthritis

  • Fibromyalgia

  • Alzheimer’s disease

  • Cachexia or wasting syndrome

  • Peripheral neuropathy

  • Intractable pain which is pain that has not responded to ordinary medications, treatment or surgical measures for more than six (6) months

  • Severe nausea

  • Seizures including without limitation those characteristic of epilepsy

  • Severe and persistent muscle spasms including without limitation those characteristic of Multiple Sclerosis (MS)

How to become our patient?

Must be a legal resident of Arkansas and provide a valid proof of residency (driver’s license or state identification).

If you are a minor (under the age of 18), you first must obtain consent from your legal guardian or parent which much must be present during visit.

 

Bring medical records from your physician documenting your current diagnoses.

Must be diagnosed with a qualifying medical condition that is approved by the Arkansas Department of Health (ADH).
